Enrollment

Members can attend two to five days per week with a minimum of four hours per day. Funding assistance may be available to those who qualify.

Program Admission Criteria

Adult Day Service members are required to:

• be able to participate in individualized and/or group activities
• be non-violent (without disruptive/combative behaviors) towards self and others
• eat independently (assistance with cutting food is acceptable)
• be partially independent in ambulation/mobility with the use of walker, crutches and/or wheelchair, and be independent or require only one person to assist in transfers
• be able to take medication per program policy
• be bowel continent
• use assistive devices such as pad, Depends, etc. if bladder incontinent
• be accompanied by a caregiver or worker prior to attendance for an on-site assessment
• provide medical and financial information prior to attendance
